Insurance Copay

You must pay the co-payment determined by your insurance company at the time of service. We can not waive your co-payment responsibility. This would be a violation of the terms of our contract with your insurance company.

If you are unable to pay your co-payment at the time of service, we will charge you a service charge when we bill you.

If you repeatedly fail to or refuse to pay your co-payment, we reserve the right to refuse services to you (medical emergencies excluded) until you pay the current charge and any outstanding balances.

It is your responsibility to know what your insurance covers and does not cover. This information is available from your insurance company or Human Resources department
