You can set up an appointment by calling our office at 708-628-0600 during the office hours listed at the right. If you would like, you can schedule a fifteen minute complimentary meeting with one of our physicans to discuss her credentials and philosophy of practice.
The first visit to any practice involves a little administrative overhead. We now have an electronic sign in system, Phreesia. On your first visit, you will need to do a complete registration with this. It may take up to fifteen minutes, depending on your history, so please arrive a little early. You will also need to sign a couple forms, downloadable below. You can get the latest copy of Acrobat Reader here.
A form saying you have read our privacy policy- A form granting us the right to treat you.
If you can't fill out these forms ahead of time, you will need your social security number, the social security number and birthdate of the primary insurance holder if that is not you, and your insurance card. Please also bring a picture ID.
Like returning patients, you MUST do the following for EVERY appointment.
- Bring your insurance card.
- Bring your copay, to be given before seeing the doctor.
- Inform us of any changes in your insurance coverage.
