We allow older teenagers (15-17) to visit us on their own after the teenager and his or her parent(s) chat with us and fill out this form.

Children Accompanied by Temporary Guardian

You can also authorize children under the age of 15 to visit us when accompanied by other adults you designate (e.g., a relative or a babysitter). Please complete this form and bring it to us before your child visits us without you.
